Health Info Authorization

Completion of this Authorization authorizes Plum Check to obtain my health information from my health care providers and third-party platforms – including information regarding HIV/AIDS test results or treatment, sexually transmitted diseases, and other communicable diseases – and use that information to create and share a summary of my relevant health statuses with other consumers using the Dating/Networking App.

I authorize Plum Check, and any vendor or service provider working on behalf of Plum Check, to obtain, use, and disclose my information as follows:

1. Obtain copies of all my medical records and information pertaining to my medical care and history for all dates of service from any and all of my health care providers, including laboratories that have processed my samples or information, and third-party platforms that may maintain my information (e.g., Apple Health). This includes medical records and information concerning HIV/AIDS and sexually transmitted and other communicable diseases;

2. Review my medical records to determine my health status with regard to HIV/AIDS and other sexually transmitted or communicable diseases; and

3. Disclose my relevant health statuses — such as HIV status (negative, positive, undetectable), date(s) of last STI screening(s), whether I have a prescription for PrEP and/or DoxyPEP, and status of vaccinations such as HPV, meningitis, Hepatitis A or B, and mpox — to other consumers using the Dating/Networking App.
